When choosing a surgeon for upper and lower eyelid surgery, it’s essential to look for a specialist with advanced training in both the aesthetics and function of the eyes.

An oculoplastic surgeon (someone who is first trained as an ophthalmologist and then completes additional fellowship training in cosmetic eyelid surgery) possesses a level of precision, anatomical understanding, and safety that is especially valuable when operating near the eyes.

It’s also crucial to find a surgeon who prioritizes natural results, takes adequate time to listen carefully, and fosters a calm, unhurried, and welcoming environment. With Dr. Lattman, you will feel heard, supported, and confident that she understands your goals and will only recommend the most appropriate course of action. Many patients also rely on genuine patient reviews, before-and-after photos, and a practice culture that values warmth, responsiveness, and thorough follow-up care.

A surgeon who embodies expertise, artistry, and compassion, qualities patients consistently attribute to Dr. Lattman, will help ensure that your experience and your results are both exceptional.

“After meeting with a few oculoplastic surgeons, I chose Dr Lattman because she listened to my concerns and had a solid plan. After meeting her caring staff, I just had a really good experience and knew this was the right choice for the upper blepharoplasty. The surgery could not have been easier, zero pain afterwards, and the anesthesia was so mild for this particular procedure that I didn’t have any grogginess or nausea after. It’s been over a year, and my results have stayed beautiful. No scars!
Life for all of us has changed. We are spending much more time on video platforms such as Zoom, Skype and FaceTime. Whether it be for social connection or work obligations we are all seeing more of ourselves. I have seen this reflected in conversations I have with patients daily in my office.

Patients often come into my office saying that they don’t like looking at their heavy upper eyelids, wrinkles or lower eyelid bags on video calls. It gives me great joy to offer both surgical and non-surgical options to improve the way my patients look and feel about themselves.

One of the most common procedures I have been performing lately is the upper eyelid blepharoplasty. This corrects heavy upper lids, removing skin that is drooping. It is a simple procedure that can be done with local anesthesia. Most patients are back to work within a few days.

I have also been combining the upper eyelid blepharoplasty with the endoscopic brow lift. This is helpful for patients who have a low brow, especially for those with heavy skin and drooping in the outer corners of the eyelids. The endoscopic brow lift gives a natural rejuvenation to the brow with hidden incisions and a fast recovery.

As an oculoplastic surgeon I spend many days talking to patients about how to improve the appearance of their eyelids. This can be a confusing topic. Patients often come to the office saying that they look “tired” all the time but don’t know exactly how to fix the problem. They tell me that no matter how much they sleep, they still wake up with dark circles and a tired appearance.

A tired appearance can be caused by several different issues. In this blog I am going to focus on the lower eyelids. Under eye bags and dark circles make the whole face look tired. No amount of sleep will help to improve the bags. Also, loose and wrinkled lower eyelid skin can give a “tired” appearance.

The non-surgical treatment for lower eyelid bags and dark circles is to use an injectable filler to smooth out the contour of the lower eyelid. I use Restylane in the lower eyelids. Filler can help to improve the couture of the lower lids and remove dark circles. Not everyone is a good candidate for lower eyelid filler. So it is important that I evaluate all aspects of the eyelid before choosing the best treatment.

The surgical treatment for under eye bags and dark circles is a lower eyelid blepharoplasty. In this procedure I remove and reposition fat pockets in the lower eyelid to improve the contour of the lid. If needed, I also tighten the deeper layers of the eyelid and trim the skin to give a smooth appearance to the eyelid. Often I will also resurface the eyelid skin with a CO2 laser to smooth out fine lines and improve the elasticity of the skin.

Men in this age group will often be reaching out to me for cosmetic evaluations for the first time. Usually I help them achieve their goals with neurotoxins (Botox, Xeomin, Dysport, Jeuveau) and fillers. Cosmetic blepharoplasty is not that common in this age group, but sometimes I will treat lower eyelid bags with a lower eyelid blepharoplasty. For men in their 20s and 30s I almost always approach these bags through a transconjunctival approach. This means that the incision is on the inside of the eyelid so there are no visible scars.

A common surgery I do for men in their 20s and 30s is the repair of congenital ptosis. This is a condition when a child is born with a droopy eyelid which stays droopy as they grow. Often only one eye is affected but sometimes both lids can be droopy. If the droop is severe enough to interfere with the development of vision then it is always repaired in the first few years of life. However, if the droop is mild, then it is usually not repaired until later.

Men in their 20s and 30s often come into my office for repair of a mild droop. Thes surgery to repair this droop or “Ptosis” is simple and done as an outpatient procedure. I tighten the muscle that opens the eye to give it more pulling power. I do this from the backside of the eyelid so there is no incision on the skin and no visible scar.

A swollen eyelid occurs when excess fluid builds up in the loose connective tissues around your eyes, Typically a swollen eyelid will resolve on its own in a day or two. But if it lasts longer than then it is important to have it evaluated and treated.

There are several reasons that you may experience a swollen eyelid. They include:

Allergies – This is the most common cause of a swollen eyelid, especially this time of year. Most allergies are due to seasonal pollen, but the swelling can also be due to a reaction to food, chemicals or other irritants that get in contact with your eyelid skin.

Conjuntivitis – More commonly known as “pink eye,” this is an infection that causes swelling, redness and discharge from the eye. It can be caused by a virus, bacteria or allergens.

Chalazion – A blockage of a tear gland in your eyelid can cause a red bump that may be tender to the touch. This may cause swelling of the affected eyelid.

Orbital Cellulitis – This is an infection of the eyelid and the tissues around the eye. This can be caused by bacteria spreading from a sinus infection or trauma to the eyelid skin. It causes redness and painful swelling of the eyelids. This is a true emergency and needs to be treated with antibiotics.

Here are some ways to keep your eyes safe and healthy.

If you wear contact lenses try switching to glasses for a while. Contact lens wearers touch their eyes more often than the average person. So consider wearing glasses for a while especially when you are out of the house. Also wearing glasses may add a layer of protections. But remember that glasses do not provide 100% security. If you feel the urge to rub your eyes, consider using a tissue instead of your fingers. Dry eyes can also lead to more rubbing, so consider adding an artificial tear drop to your routine. There are also many good over the counter allergy drops that can help to stop the itch.
The eyelid skin is often the first area on the face to show signs of aging. I see many patients who are bothered by the skin under their eyes, but are not yet candidates for cosmetic eyelid surgery. Many patients often ask what they can do to rejuvenate the delicate eyelid skin, and how they can prevent aging.

First, the skin around your eyes is thinner and more delicate than the skin on any other part of your body. Because the eyelid skin is in constant motion, blinking, smiling etc, it gets a lot of wear and tear. Also the eyelid skin is exposed to UV rays more than other areas of the body. Genetic predisposition can also lead to early aging around the eyes.

Common issues related to the eyelid skin are, fine lines, discoloration, dark circles and puffiness. No matter what your age, it is never too soon, or too late to start caring for your eyelid skin. Here are a few of my favorite tips:

The procedure corrects droopy, sagging, puffy eyelids that have been affected by aging, gravity, and genetics. Preparing for your recovery can make that first week a breeze.
As an eyelid specialist, I see patients every day who are bothered by aging changes of their eyelids. I am often asked, “What can I do to rejuvenate my eyes?” Some of my patients come to see me with a procedure in mind and others are interested in simply knowing what’s possible. Below are surgical and non-surgical options for improving the appearance of the eyelids:

Cosmetic Blepharoplasty: Cosmetic Blepharoplasty is a surgical procedure where the upper and/or lower eyelids are rejuvenated. This involves the removal of excess skin and the tightening of the underlying muscle. Fat pockets are also trimmed and/or repositioned to improve the contour of the eyelid.

Injectable Filler to the Lower Eyelids: Injectable fillers such as Restylane can be injected into depressions of the lower eyelid to improve contour. This can help to improve the appearance of lower eyelid bags and dark circles.

Laser Skin Resurfacing: In my office I use a fractionated CO2 laser to improve the appearance of the eyelid skin. This laser reduces fine lines, gives the skin a firmer, smoother appearance, improves scars, reduces sun damage such as brown spots and reduces skin blotches. The laser can be used to treat just the eyelid skin or the whole face.

The upper lid appearance is very much impacted by the eyebrow position. Eyebrows that have fallen with age can give a tired and heavy appearance to the upper eyelids. An endoscopic brow lift surgically repositions the eyebrows back to a youthful and natural position.

Botox, Xeomin, Dysport (Neuromodulators): When strategically placed, neuromodulators can help to rejuvenate the eyelids.
