Neurogenic Ptosis

Neurogenic ptosis occurs when a problem in the nerves controlling the eyelid muscles prevents normal lifting. Conditions such as Horner’s syndrome, third cranial nerve palsy, or myasthenia gravis can lead to this form of drooping.

Typical signs:

Sudden eyelid drooping, sometimes with double vision

Unequal pupils or decreased eye movement

Fatigue that worsens throughout the day

The first step is identifying and treating the underlying neurological condition. Once the cause is stabilized, surgical or nonsurgical correction may be considered. As both an oculoplastic surgeon and trained ophthalmologist, Dr. Lattman is uniquely qualified to evaluate these complex cases safely and comprehensively.

Choosing the right surgeon for eyelid rejuvenation will shape both your results and your experience. In NYC, Dr. Jessica Lattman focuses on natural, rested eyes without a “done” look. Her upper blepharoplasty surgeries are precise, clear, and tailored to restore a refreshed, youthful look to your eyes.

1) Expertise in NYC Upper Blepharoplasty

Q: What experience does Dr. Jessica Lattman have in upper blepharoplasty?
Upper blepharoplasty is a core focus of Dr. Lattman’s oculoplastic practice. She blends aesthetic judgment with expertise in eyelid anatomy to protect function while refreshing appearance. This blends the best of both worlds for your upper blepharoplasty with:

Focus on conservative tissue removal to avoid hollowing

Attention to eyelid crease height and symmetry for a natural lid fold

Judgment about skin-only versus skin-muscle approaches

Precise fat pad contouring to soften heaviness without over-resection

This depth of focus helps upper blepharoplasty look effortless and awake, not surgical. Dr. Lattman’s goal is to help you understand each choice and how it supports refreshed, balanced eyes.

2) Customized Planning for Your Upper Blepharoplasty

Q: How will Dr. Lattman tailor upper blepharoplasty to your eyes and goals?
Your evaluation will focus on skin excess, crease position, brow support, and dry-eye tendencies. From there, Dr. Lattman will develop a plan that aligns with your lifestyle and desired downtime. Your upper blepharoplasty plan may include:

Matching your ideal crease height and natural lid contour

Skin pinch or skin-muscle flap based on tissue quality and fold definition

Targeted fat contouring where fullness causes hooding

Coordination with brow position to avoid over-tightening the lid

Timing that fits work, events, and contact lens use

You will leave with a step-by-step roadmap for upper blepharoplasty that fits your schedule and the level of change you want to see.

3) Incision Design, Tissue Management, and Scar Strategy in Upper Blepharoplasty

Q: Which techniques shape upper blepharoplasty results and crease definition?
Incisions are carefully placed in the future crease so scars are nearly invisible, blending into a fine line in the lid fold. Closure is layered to support smooth drape and stable definition. Dr. Lattman plans the upper blepharoplasty steps from:

Crease-line incisions placed to hide in your natural fold

Gentle tissue handling to reduce swelling and bruising

Secure fixation to maintain the planned crease height

Ointment, eye care, and a simple at-home routine you will follow

Early scar care and long-term fading guidance as healing matures

These details protect expression and blink function while refining the fold. Dr. Lattman aims for an upper blepharoplasty that looks natural at rest and in motion.

4) Upper Blepharoplasty Recovery and Results Timeline in NYC

Q: What will recovery look like after upper blepharoplasty with Dr. Lattman?
Recovery is straightforward and mapped out in plain language. You will receive written instructions, early check-ins, and clear milestones. During upper blepharoplasty recovery, you can expect:

Head elevation and cool compresses during the first 48 hours

Temporary dryness, tightness, and light sensitivity as swelling peaks

Suture removal typically occuring around the end of week one

Makeup and contacts resuming only when advised at follow-up

Return to social activities as bruising fades over one to two weeks

Most patients notice their eyes look more open as swelling settles in the second week. Full refinement of upper blepharoplasty continues over the following weeks as the crease softens and the line fades into the fold.

5) Oculoplastic Safety and Patient-First Support for Upper Blepharoplasty

Q: What will your experience be like during upper blepharoplasty with Dr. Lattman?
Dr. Lattman and her team offer warm, organized, and efficient care. Visits are thorough yet never rushed, with eyelid-specific guidance before and after surgery. Throughout upper blepharoplasty, you can expect:

Candidacy screening that considers dry eye, medications, and healing history

Local anesthesia with comfort measures discussed in detail

A garment-free recovery with simple eye care you can manage at home

Realistic downtime planning that fits work and events

Regular follow-ups that track crease symmetry, swelling, and comfort

You will feel heard and supported from consultation to final results, with a focus on precision and subtlety for your upper blepharoplasty. Dr. Jessica Lattman brings eyelid-focused expertise, meticulous technique, and calm guidance to upper blepharoplasty in NYC. Traditionally performed under sedation in an operating room, upper blepharoplasty can now be safely and comfortably performed awake under local anesthesia, thanks to advances in surgical precision and patient care.

What Is Awake Upper Blepharoplasty?

Awake upper blepharoplasty is a procedure performed under local anesthesia to remove excess skin, muscle, or fat from the upper eyelids. The technique addresses both cosmetic and functional concerns, such as drooping lids or heaviness, while maintaining the patient’s natural eye shape and expression.

Unlike traditional surgery under full sedation, the awake method allows patients to remain conscious and relaxed throughout the procedure. This in-office approach reduces downtime, minimizes risk, and provides an enhanced sense of comfort and control.

As detailed in Dr. Lattman’s recently published article in ScienceDirect, careful patient selection and pre-operative evaluation are key to ensuring a safe and positive experience. During the consultation process, Dr. Lattman takes the time to assess each patient’s anatomy, comfort level, and goals before recommending the best approach for their needs.

The Advantages of Staying Awake During Eyelid Surgery

For many patients, the idea of remaining awake during surgery sounds surprising at first, but the benefits are significant.

1. Lower Risk, Greater Safety
Awake blepharoplasty uses local anesthesia provides targeted numbing around the eyelids, ensuring a completely pain-free experience.

2. Faster Recovery and Less Downtime
Since there is no systemic sedation, recovery begins immediately after surgery. Most patients return home shortly afterward and resume light activity within a few days.

3. Comfort in a Familiar Setting
The procedure takes place in Dr. Lattman’s fully accredited, state-of-the-art surgical center, where patients are surrounded by a highly trained, compassionate team they already know.

Patients often describe the experience as “surprisingly easy” and say they appreciate how present and calm Dr. Lattman keeps them throughout. Her gentle, reassuring communication style helps even nervous patients feel safe and confident.

How NYC Awake Upper Eyelid Surgery Works

The process begins with meticulous pre-operative planning. While seated upright, patients have delicate markings placed to outline the natural eyelid crease. The area is then numbed using local anesthesia, which takes effect within minutes.

Once fully comfortable, Dr. Lattman removes the precise amount of excess skin and fat needed to restore a smooth, lifted contour to the upper eyelids. Her technique, honed through more than 27 years as an oculoplastic surgeon, prioritizes both aesthetic beauty and functional safety to ensure the eyes close properly and the shape remains natural.

Tiny sutures are placed along the eyelid crease, where they are virtually invisible. The entire procedure typically takes less than an hour, and patients are able to return home the same day.

Who Is a Good Candidate for Awake Eyelid Surgery in NYC?

Awake upper blepharoplasty is ideal for patients with mild to moderate eyelid laxity who want a rejuvenated, natural look without the downtime of general anesthesia.

Good candidates include individuals who:

Can remain still and relaxed during the procedure

Have minimal anxiety about being awake

Are in good overall health

Have realistic expectations for their results

During your consultation, Dr. Lattman will carefully evaluate your anatomy, medical history, and comfort level to determine whether an awake approach is best for you. If mild sedation or an alternative plan would ensure a better experience, she will discuss those options openly and honestly.

Recovery After Awake Blepharoplasty

Recovery from awake upper blepharoplasty is typically smooth and manageable. Most patients experience only mild swelling and bruising for the first several days. Icing, gentle wound care, and rest are recommended during this time.

Patients often return to light activity within three to five days and to social or work settings in about a week. Since there is no general anesthesia involved, many find they feel alert and clear-headed immediately after surgery.

Dr. Lattman and her team provide personalized follow-up care and remain available every step of the way. As one patient shared, “I never felt alone during recovery; Dr. Lattman checked in personally and made sure I was comfortable every step of the way.”

Upper Eyelid Surgery While Awake – Results That Look Effortless

Awake blepharoplasty achieves the same level of artistry and refinement as traditional eyelid surgery, often with faster recovery and less disruption. The outcome is subtle yet transformative: eyes that look brighter, smoother, and more rested, while remaining unmistakably yours.

Dr. Lattman’s patients often describe hearing compliments like, “You look amazing; did you just get back from vacation?” The change is noticeable, but never obvious. Her results speak to her philosophy: rejuvenation that restores confidence, not recognition.

Awake Eyelid Surgery in NYC – FAQs

How long does awake blepharoplasty take?
Most awake upper eyelid surgeries take about 45 minutes to an hour, depending on the complexity of the case.

Will I be able to drive myself home after awake eyelid surgery?
If you receive a mild sedative, such as a benzodiazepine, you’ll need someone to drive you home. Patients who have only local anesthesia and feel completely comfortable can usually go home without an escort.

Can I combine upper blepharoplasty with other treatments?
Yes. Dr. Lattman may combine upper blepharoplasty with complementary procedures, such as laser skin resurfacing, brow lift, or non-surgical treatments, for a more comprehensive result.

When can I return to wearing contact lenses?
Patients should wait about two weeks before resuming contact lens wear to allow the eyelids to heal fully.

Are the results permanent?
While eyelid aging continues naturally over time, most patients enjoy their results for 15 to 20 years or longer. Maintaining good skin care and overall health helps preserve the refreshed appearance.

This is a case study of a 24 year old woman who was born with a droopy right eyelid. As you can see in the “before” photo, her right eyelid is sitting lower than her left by about 2 millimeters. The ptosis has been present her whole life, but it has started to bother her over the past several years. The patient was given upneeq by her general ophthalmologist which helped the droop, but the upneeq would wear off as the day went on and she was looking for a permanent solution to the asymmetry.

I performed an internal ptosis repair on her right upper eyelid. The procedure was done in my surgery center and took about 30 minutes. She went home an hour later and was back at work the next day. There was some mild swelling and bruising that resolved over the next week. The procedure was done completely from the back side of there eyelid, so there was no visible incision on the skin and no scar. Ptosis surgery is delicate and requires specialized training as an oculoplastic surgeon and years of experience.

The after photo was taken about 6 weeks after the surgery. You can see perfect symmetry with the height and contour of the eyelid. She is thrilled with the beautiful result and excited to enjoy her new eyelid!

Life for all of us has changed. We are spending much more time on video platforms such as Zoom, Skype and FaceTime. Whether it be for social connection or work obligations we are all seeing more of ourselves. I have seen this reflected in conversations I have with patients daily in my office.

Patients often come into my office saying that they don’t like looking at their heavy upper eyelids, wrinkles or lower eyelid bags on video calls. It gives me great joy to offer both surgical and non-surgical options to improve the way my patients look and feel about themselves.

One of the most common procedures I have been performing lately is the upper eyelid blepharoplasty. This corrects heavy upper lids, removing skin that is drooping. It is a simple procedure that can be done with local anesthesia. Most patients are back to work within a few days.

I have also been combining the upper eyelid blepharoplasty with the endoscopic brow lift. This is helpful for patients who have a low brow, especially for those with heavy skin and drooping in the outer corners of the eyelids. The endoscopic brow lift gives a natural rejuvenation to the brow with hidden incisions and a fast recovery.

As an oculoplastic surgeon I spend many days talking to patients about how to improve the appearance of their eyelids. This can be a confusing topic. Patients often come to the office saying that they look “tired” all the time but don’t know exactly how to fix the problem. They tell me that no matter how much they sleep, they still wake up with dark circles and a tired appearance.

A tired appearance can be caused by several different issues. In this blog I am going to focus on the lower eyelids. Under eye bags and dark circles make the whole face look tired. No amount of sleep will help to improve the bags. Also, loose and wrinkled lower eyelid skin can give a “tired” appearance.

The non-surgical treatment for lower eyelid bags and dark circles is to use an injectable filler to smooth out the contour of the lower eyelid. I use Restylane in the lower eyelids. Filler can help to improve the couture of the lower lids and remove dark circles. Not everyone is a good candidate for lower eyelid filler. So it is important that I evaluate all aspects of the eyelid before choosing the best treatment.

The surgical treatment for under eye bags and dark circles is a lower eyelid blepharoplasty. In this procedure I remove and reposition fat pockets in the lower eyelid to improve the contour of the lid. If needed, I also tighten the deeper layers of the eyelid and trim the skin to give a smooth appearance to the eyelid. Often I will also resurface the eyelid skin with a CO2 laser to smooth out fine lines and improve the elasticity of the skin.

Even after a good night’s sleep, his droopy eyelids still made him look tired which was affecting his work. It began to bother him more recently with the constant Zoom calls and close-up video.

I performed a joint case with Dr. Benjamin Paul. I did a bilateral ptosis repair and Dr. Paul performed a chin implant, neck liposuction and buccal fat removal.

In the post op views you can see the patient’s eyelids are in a great position. He looks awake and the eyelids are perfectly symmetrical. I maintained his eyelid shape and natural appearance.

Dr. Paul’s work gave him a defined jawline and more of a V-shape to his face. Overall, the patient was thrilled and returned to work and zoom calls with more confidence and a bright-eyed appearance.
