Wrinkle-smoothing botulinum toxin type A injections are the most common non-invasive aesthetic procedure in the U.S. While they are commonly called “Botox,” there are actually four different injectables. The four neurotoxins FDA-approved for cosmetic use are Botox®, Dysport®, Jeuveau®, and Xeomin®.

While each neurotoxin is slightly different, some patients respond better to one over the other. Knowing this, I keep all 4 on hand in my office and choose the best one based on the patient and their goals. Most doctors and patients refer to neurotoxins as “Botox,” much like we call adhesive bandages, band-aids and tissues, Kleenex even though there are the 4 different companies that now manufacture botulinum toxin type A.

As an Oculoplastic surgeon, I have a deep understanding of the anatomy of the eyelids and how the muscles around the eye work. This knowledge of the anatomy of the eyelids, eyebrows and upper face allows me to expertly use botox to achieve my patient’s goals.

When I meet with a new patient who is requesting Botox, I first evaluate their entire face. We then discuss the specific areas that brought them to the office, and what they would like to do to correct them. Then I realistically explain what Botox can achieve.

The main areas I treat with Botox are the forehead, glabella (from lines), around the eyes (crows feet), masseter muscle (jawline) and mouth lines. Botox can be used to smooth dynamic wrinkles, lift the eyebrows, and smooth mouth lines. I also use it to relax the masseter muscle which can help with teeth clenching, migraines and improves an overly square or thick jawline.

The laser can be done at the same time as surgery so that there is one healing time. The laser can also be done on its own.

In my office center, I use the Lutronic eC02 fractional laser. This laser works by creating tiny micro wounds, leaving bridges of healthy skin, thus treating a “fraction” of the skin’s surface. This allows quick healing times with great results.

The laser reduces imperfections in the skin and promotes collagen regeneration for long term wrinkle reduction.
I am asked all the time what is the “right” age to start Botox treatments. The answer depends on each individual. The youngest patient I have treated with Botox was 25 at the time. Starting early can help to prevent permanent wrinkles from forming. Once a frown line or forehead line is etched into your skin when you are not expressing, it can be challenging to treat with Botox alone.

Many of my young patients are nervous at first that they will look overdone. Some are worried that Botox will make their face look frozen. This is absolutely not the case. In the right hands, Botox treatments look soft, natural, and overall make the face look more rested and youthful. However, the final result does depend on the experience and technique of the injector.

As an oculoplastic surgeon, I understand the dynamic relationship of the muscles around the eyes and the upper face. I can expertly use Botox to raise the brows (liquid brow lift) and also to soften frown and smile lines, all while keeping the face looking natural and untouched. I understand how it is essential that the facial muscles are balanced so that the brow remains in a natural position, and nothing looks “done.”

Torn earlobes are a common problem. Years of wearing heavy earrings, or trauma to the earlobe, can cause your ear-piercing hole to stretch and become deformed. Stretched or torn earlobes make it difficult to keep earrings in and don’t look great.

Surgical repair of a torn earlobe is a quick and simple procedure that is done in the office under local anesthesia. I use advanced surgical techniques to repair the damaged hole and restore a pleasing shape to the earlobe. Some holes are torn all the way through, and others are just stretched out— but either way, the fix is easy.

For the procedure I numb the earlobe with a local anesthetic. Then, I remove the damaged area and suture the healthy tissue together. There is no downtime after the procedure. Usually, patients do both sides at the same time. Seven to ten days after the procedure, the sutures are removed. Patients usually return to the office in about six weeks to have the ear re-pierced.

Learn more about Earlobe Rejuvenation and Repair.

Call us at (212) 832-5456, or request online that we call you back by submitting our online request form here.

This is achieved with a completely natural look. Patients often express fear of looking frozen or startled. I tell all my patients that Botox is simply a tool. It takes skill and experience to use this tool properly to achieve natural results without the telltale signs of having been treated.

I love taking care of my Botox patients. It’s always a treat to see them every 3–4 months for their treatment. I have many patients whom I have been taking care of since I started my practice 21 years ago. Over time, as these patients continue to age, Botox patients often become surgical patients.

Upper lid blepharoplasty (often combined with an endoscopic brow lift) is then an excellent option to correct these issues.
Posted December 13th, 2016

A: This is a great question. The eyes are often the first area of the face to show signs of aging, and sometimes patients want to look less tired but are not ready to take on surgery. So here are some non-surgical options to help you look your best.

Botox for smile lines, frown lines and forehead wrinkles: Botox allows wrinkled skin to regenerate and also prevents further wrinkling. Botox can also be used, when strategically placed, to lift the eyebrows in a natural way, so that the skin on the upper eyelid appears less heavy.

Laser Skin resurfacing: Winter is an excellent time for a laser resurfacing. The laser removes the top layer of damaged cells and allows for new start. It also stimulates deeper collagen to regenerate and tighten the skin. On eyelid skin, I sees particularly great results from laser resurfacing. The laser I like best for the eyelids is a combination of a fractionated CO2 laser followed by an erbium laser. Healing takes about five days, but it’s really a great solution for wrinkled and sun-damaged eyelid skin.

Platelet Rich Plasma (PRP) is another approach to rejuvenate the eyelid skin that is non surgical. I inject PRP into the eyelid skin using a device called the Aquagold fine touch delivery system. This device allows me to deliver PRP deep into the eyelid skin. This treatment stimulates the body’s own collagen to fill in lines and rejuvenate the skin. This is a nice option because there is no downtime. You can have the procedure and go right back to normal activities.

Injectable fillers, such as Restylane, are a great option to fill in hollows around the eye area. This works best for contour issues on the lower lid. I also use filler to add volume to hollow upper lids.
