Can Micro Fat Grafting Be Combined with Upper and Lower Blepharoplasty?

Upper blepharoplasty and lower blepharoplasty are highly effective procedures for addressing heaviness of the upper lids and puffiness beneath the eyes. In some cases, however, there is also a degree of volume loss that contributes to a tired or hollow appearance.

Micro fat grafting can be combined with upper and lower blepharoplasty for select patients to create a more balanced and refreshed result. The key is understanding when it is appropriate and how it can enhance the results of a lower blepharoplasty or used on its own to improve the contour of the lower eyelid.

What Are Upper and Lower Blepharoplasty?

Upper blepharoplasty focuses on removing and reshaping excess skin on the upper eyelids that can create a heavy, tired appearance. Patients often feel as they age that their eyes look less open and that excess skin hangs down over the eyelid. The goal of an upper blepharoplasty is to restore a more lifted, natural contour without changing the fundamental shape of the eye.

Lower blepharoplasty addresses aging changes on the lower eyelids beneath the eye. These changes can be puffiness of the lower lids, sagging skin, hollowness, and poor skin texture. The goal of a lower lid blepharoplasty is to restore a smooth contour to the lower lid, eliminating bags, hollows and tightening excess skin.

Where Micro Fat Grafting Fits In

For some patients, the hollowing beneath the eyes can be extremely bothersome. The hollow areas can create shadows that contribute to a tired appearance.

Micro fat grafting uses a small amount of a patient’s own fat to restore volume under the eyes. Microfat can soften hollowing and improve the overall contour of the lower lid. Microfat can also be used in the temples, jowls, cheeks, and around the mouth for an overall facial rejuvenation.

The decision to include fat grafting with cosmetic blepharoplasty or on its own is based on a careful evaluation of each individual’s anatomy and goals.

Can Eyelid Surgery and Micro Fat Grafting Be Performed Together?

Yes, micro fat grafting is often performed at the same time as upper and lower blepharoplasty. When indicated, combining procedures allows for a more comprehensive approach to rejuvenation.

Each patient’s treatment plan is individualized. Some patients benefit from addressing only excess skin or puffiness, while others may benefit from a combination approach that includes volume restoration.

Droopy Eyebrows?

I see many patients in my practice that are bothered by tired looking eyes and heavy, drooping eyebrows. When I mention an eyebrow lift, many patients ask, “What is an eyebrow lift!?” Eyebrows can be a confusing topic.

The options to treat drooping eyebrows are varied, so here I will explain the options, what to expect, and how to choose the best brow lift surgeon.

Why do brows droop?

As we age the muscles of facial expression, especially the frown and smile muscles pull down on the eyebrows. After decades of smiling and frowning, the eyebrows can begin to drop. A low brow can add to a heavy appearing upper eyelid, and also can lead to hooding of the eyelid in the outer corners that extends past the corner of the eye. My patients with low brows often say they look tired and angry. They also complain of forehead wrinkles from constantly using their forehead muscles to lift up their drooping brows.

How can low brows be fixed?

There are several great ways to address heavy, low eyebrows. The simplest is to do a “liquid brow lift.” This is when I use a neuromodulator (Botox, Xeomin, Dysport, Jeuveau) to lift the brow, When properly done, neuromodulators can adjust the dynamics of the muscles to result in a lifting of the brow. This is a great option for mild cases of droopy brows, but is not a perfect solution for everybody.

In patients who need more help lifting the brows, beyond what a “liquid lift” can offer, the endoscopic brow lift is a great option. In an endoscopic brow lift, I surgically lift the brows, putting them back in a natural position. Often a brow lift is combined with an upper lid blepharoplasty for the best results.

I love performing endoscopic brow lifts, because I can customize the lift for each patient. Not all drooping brows are the same. The surgical brow lift allows me to lift where and how much each patient needs. The procedure is done “endoscopically” which means I use an operating telescope (similar to sinus surgery) and thus only need to make tiny incisions which are hidden behind the hairline.

How do you choose the best eyebrow surgeon?

An endoscopic brow lift is a procedure that should be done by a sub-specialist. It is importante that your surgeon is extremely familiar with the procedure and does a lot of them. As an oculoplastic surgeon I only perform surgery on the eyes and eyebrows. Endoscopic brow lifts are one of the most common surgeries I perform.
