Case Study – Endoscopic Brow Lift and Upper Lid Blepharoplasty

This is a case study of a 65 year old woman who came to see me because she was bothered by her heavy upper lids. She wanted to relieve the fullness of the upper lids but also wanted to look 100% natural.

I discussed with her the option of doing an upper lid blepharoplasty combined with an endoscopic brow lift. I explained that this would give her the most natural looking result while fully rejuvenating the upper lids and brow region. She was nervous, as this was her first experience with cosmetic surgery, but chose to proceed with the proposed surgery.

This picture is of her 6 months after her surgery. She is thrilled with her refreshed appearance and the improved symmetry of her eyebrows. Cosmetic eyelid and brow surgery both address aging changes and asymmetry. She recently told me that this was “the best gift,” she could have ever given herself and not only looks so much better, but she also feels lighter and less tired. This natural result will last a lifetime!

Combining Upper Eyelid Surgery with Endoscopic Browlift

Often, patients come to me unsure what they need or want. They have a general idea that they want to look less tired, younger, natural, not “done,” but that’s all they are able to express. I love to explore their faces with them and offer suggestions on how I can help them look more like themselves.

Often while examining the upper eyelids, we notice together that low eyebrows are contributing to their tired look. While some patients simply have excess skin on their upper lids, others have excess skin and a low brow. I will take my finger and gently lift their brows to show them how a brow lift can improve their appearance and still look natural.

I think brow lifts have gotten a bad reputation because the older techniques produced a more severe result and required a large incision across the hairline.

I perform my browlifts using an endoscopic approach. This is done through 4 small incisions behind the hairline. I use an operating telescope to lift the brow through these small incisions. Recovery is quick. The results are natural, and in the right patients, an endoscopic brow lift is an important part of rejuvenating the upper eyelids.

What is a Brow Lift and how do I know if I need one?

Looking young and healthy is important for a personal sense of well being. In our society today, one’s appearance can be essential both professionally and socially. The eyebrows and forehead are distinct features of the face. When aesthetically pleasing, they can convey a sense of restfulness, attractiveness and health. However, with drooping of the eyebrows and contraction of the forehead muscle, the upper face may appear sad, tired, or angry. Furthermore, a sagging forehead can create heavy upper eyelids, browache and functional eyelid problems.

Before undertaking forehead rejuvenation, a careful evaluation of brow position and contour, forehead wrinkles, hairline and upper eyelids is essential. After physical examination and thorough discussion of the your goals and expectations, the appropriate procedure can be undertaken.

The technique I use for a Brow Lift is the endoscopic approach. The endoscope is placed below the skin through a small incision hidden behind the hairline, and then tunneled down to the brows. Using the endoscope, I can lift the entire forehead is and secure it in its new position. The main advantages of the endoscopic technique are smaller incisions which are easily hidden even in thinning hair and a quick recovery time.

Anyone, regardless of age can be a candidate for cosmetic upper face rejuvenation. The main goal of cosmetic surgery is to enhance appearance, and sometimes correct a functional problem. The decision to undergo the procedure is purely a personal one.

When to undergo surgery is also a personal decision, however, many patients do not realize how much their eyebrows droop because it happens gradually. The body then compensates for the droopiness by holding the eyebrows up using the forehead muscles, resulting in raised eyebrows and forehead wrinkles.
