This 54-year-old woman had been bothered by her under-eye bags for years. She finally couldn’t take them anymore and decided to have cosmetic eyelid surgery. I performed an upper and lower eyelid blepharoplasty on her 6 weeks ago. She is absolutely thrilled with her new eyelids.

I used a subciliary approach to do the surgery of her lower lids. This allowed me to recontour the lower eyelids and also tighten the skin and deeper muscle to give her a perfectly smooth lower eyelid with a natural look.

Cosmetic Surgery in the Era of COVID

I have many patients and friends asking me, “Is it safe to have cosmetic eyelid surgery, Botox and filler in COVID times?” My universal answer is, Yes! Now is a great time to have cosmetic eyelid surgery. In fact, I have seen an increase in patients coming in to discuss improving the appearance of their eyelids and having surgery.

In some ways, this is a perfect time to have cosmetic eyelid surgery. There are no parties, events or in-person work commitments. Most vacations are being taken at home or only a car ride away. So taking a week or so off to recover has never been easier.

I am seeing many patients in the office that also have been looking at their eyes on Zoom calls and now are finally ready to do something about their eyelid bags or tired-looking eyelids. Also, as the rest of their face is covered by a mask, the eyes are now truly the only facial feature that others are seeing. So everyone wants them looking their best.

My patients have truly appreciated all the steps I have been taking in the office and in my private surgery center to ensure their safety. We have all the usual precautions: temperature checks, screening questions, social distancing and thorough cleaning between patients, etc. But we also have our entire staff COVID tested once a week, and all patients are tested before surgery. With these precautions we have maintained a COVID free environment in our office and surgery center.

It has given me great joy, especially in the era of COVID, to be in the business of making my patients look and feel better.

How to achieve natural results with cosmetic eyelid surgery

The modern aesthetic for eyelid lifts is a natural look. Patients want to appear refreshed after surgery, not pulled and tucked. The biggest concern I hear from my patients is that they will look “done.” So how can you be assured that your result will be just right?

First it’s important to find a surgeon that understands your aesthetic sensibility. I tell my patients that their surgery is all about them and their goals. I want them to see me as a tool to guide them and help them achieve exactly what they are hoping for. I find it is essential to listen closely to my patients, so I can be sure to completely understand what they want to achieve with their surgery.

In our initial consultation I explain what we can achieve with the various surgical procedures that I offer. Whether it is a cosmetic blepharoplasty, endoscopic brow lift or a ptosis repair, it is important for the patient to understand how surgery will improve their appearance. It is often helpful to look at before and after photos of patients that I have operated on. This helps to envision what the surgery will achieve.

Cosmetic Eyelid Surgery FAQs

I thought it would be helpful to list some of the most asked questions I get on a day to day basis when consulting with new patients about cosmetic eyelid surgery. Here are a few and more to follow in future blog posts:

How long does it take to recover from an upper eyelid blepharoplasty?

For most patients, an upper eyelid blepharoplasty is a relatively easy procedure and recovery. There is some slight discomfort after the surgery. Most of my patients manage the discomfort with Tylenol. The day of the surgery you will need to ice for about 20 minutes per hour, but otherwise you can be up and about at home. You can be on your phone or watching TV. Stitches come out one week after surgery. You will have some mild bruising and swelling that will be mostly gone after 7 days.

What type of anesthesia is used for cosmetic blepharoplasty?

For a cosmetic blepharoplasty there are several choices for anesthesia. In my private surgical center we offer light IV sedation to general anesthesia. For the upper eyelids, some patients choose to be completely awake and others prefer IV sedation. When I am performing a lower lid blepharoplasty, general anesthesia is used to keep my patients safe and comfortable.

What results can I expect after a cosmetic blepharoplasty?

You can expect your eyes to look refreshed, less tired and completely natural.

What is the best time of year to have cosmetic eyelid surgery?

That is a personal decision. Some of my patients like the winter months, because they are less active with outdoor activities and can more easily take the time to recover. Other patients like the summer months because their kids are away at sleep away camp and they have fewer scheduled activities.

Coronavirus and Your Eyes

How can Coronavirus affect your eyes?

The coronavirus can spread through your eyes, just as it does through your nose or mouth. When someone who has coronavirus coughs or sneezes, virus particles spray from their mouth or nose onto your face. You may then breathe these tiny droplets in through your nose or mouth. The virus can also enter your body through your eyes. You can also become infected by touching your eyes after touching something that has the virus on it.

It may be possible for the viru to cause pink eye infection (conjunctivitis), but this is extremely rare. At this time, we do not believe that coronavirus causes chalazions or other eyelid conditions.

Protecting Your Eyes

Covering your eyes with glasses or a face shield can help to slow the spread of coronavirus. Here are some ways to keep your eyes safe and healthy.

If you wear contact lenses try switching to glasses for a while. Contact lens wearers touch their eyes more often than the average person. So consider wearing glasses for a while especially when you are out of the house. Also wearing glasses may add a layer of protections. But remember that glasses do not provide 100% security. If you are caring for a sick person consider wearing goggles or a face shield.

Avoid rubbing your eyes. Especially as allergy season gets into full swing, it can be hard to not rub your eyes. If you feel the urge to rub your eyes, consider using a tissue instead of your fingers. Dry eyes can also lead to more rubbing, so consider adding an artificial tear drop to your routine. There are also many good over the counter allergy drops that can help to stop the itch. If you must touch your eyes, first wash your hands with soap and water, and then wash them again after touching them.

What is the best time of year to have cosmetic eyelid surgery?

I am often asked by patients what is the best time of year to have cosmetic eyelid surgery. There are pros and cons to each time of year. But what I universally believe is that it should be done at a time of year when you can block out time from work and other responsibilities to give yourself time to rest and heal.

The winter is often a good time to have cosmetic blepharoplasty because most people are less active and can hunker down for a few days while they recover. I have a lot of patients who don’t want to give up time out on the golf course or swimming at the beach. So for them the winter is a great time.

For others the summer is the ideal time. School and work schedules tend to slow down and the evidence of surgery can be easily covered with a pair of sunglasses. I have a lot of moms who do their surgery while their kids are off at sleepaway camp or other summer adventures.

If you are combining your surgery with a laser skin resurfacing I would suggest doing it at a time when you will not be in direct sun, so fall and winter are the best times for that.

As an oculoplastic surgeon, cosmetic eyelid surgery is the most common surgical procedure that I perform. Over my 21 years in practice, I have performed thousands of cosmetic blepharoplasties and endoscopic brow lifts. Here are some of the most frequently asked questions.

Will I still need to do Botox after eyelid surgery?

Yes, you will still need to have Botox treatments after surgery. Botox and surgery treat different issues. Botox treats dynamic wrinkles, meaning that it smooths wrinkles that appear when you make facial expressions, smile, or frown. Surgery addresses loose skin, hollows, bags, and other structural issues. Surgery and Botox go hand in hand.

Can I do a laser skin resurfacing at the same time as my cosmetic blepharoplasty surgery?

Yes, I often combine CO₂ laser resurfacing of either of just the eyelids or the entire face with a cosmetic blepharoplasty. Many patients like to have one recovery period and the laser can enhance the result of the surgery.

I have ptosis (a droopy eyelid). Can ptosis repair and cosmetic blepharoplasty be done at the same time?

Yes, I often combine ptosis repair and cosmetic blepharoplasty.

Can I have an endoscopic brow lift at the same time as my cosmetic blepharoplasty?

Yes, I often perform endoscopic brow lifts and cosmetic blepharoplasty at the same time. In patients with a heavy brow, lifting the brow with surgery can improve the appearance of the upper lid, but often an upper lid blepharoplasty is also needed to give the fully rejuvenated look. Lower lid blepharoplasty is also often done at the same time.

What is the right age to have eyelid surgery?

The answer is different for everyone! I see many patients in their 20s who have had lower lid bags since their teens. For these patients, mid to late 20s can be the right time to have these bags removed.

I see patients in their early 30s who are beginning to notice frown lines and crow’s feet. For them, it may be the right time to start getting Botox.

I also see patients in their late 40s who have never considered cosmetic surgery but no longer want to look tired. They feel great, but what they see in the mirror does not reflect how they feel. For these patients, late 40s may be the right time for cosmetic eyelid surgery.

And then I see many patients in their 50s, 60s, 70s and even 80s who want to look refreshed and less tired. So cosmetic eyelid surgery can be a good idea at many different ages.

How Diet and Lifestyle Affects Recovery

Most of my patients who have cosmetic eyelid surgery are active and want to return to their routines as quickly as possible. I coach my patients before and after surgery with suggestions for diet and lifestyle choices that can speed up recovery.

In the weeks before surgery, I tell my patients to get plenty of sleep, avoid excess alcohol, avoid sugar and processed food, and to exercise. This helps ensure that their bodies will be optimized to heal and recover after cosmetic eyelid surgery.

For the two weeks after surgery, activity is limited, so I encourage patients to continue healthy eating and good sleep habits. Most patients can return to a normal routine (excluding heavy exercise) after a few days.

Looking On The Outside How You Feel On The Inside

I meet patients every day in my practice who tell me they want to look on the outside how they feel on the inside.

They explain how they go to the gym, eat well, and stay out of the sun, but still feel different from the image they see in the mirror.

I get it! As a 50-year-old woman, mom of 3 kids, I understand wanting to feel your best and look your best. As an oculoplastic surgeon, I have dedicated my career to helping people look good in a safe and nurturing environment. I take care of my patients as I would want to be taken care of.

While I always honor my patient’s privacy, I think it is helpful for me to be very upfront about what I do cosmetically. On a regular basis I get Botox, fillers and light laser treatments on my skin. In January of 2017, I had a nose job, performed by my husband and office partner, Dr. David Rosenberg. I plan to continue to “nip and tuck” as I age.

I think sharing this will help my patients feel more comfortable talking to me about their cosmetic goals. I also want to show them that it is possible to undergo treatments and surgeries and while still looking natural. My goal is for my patients to look their best, feel their best, and maintain their own natural appearance.
