Cosmetic Surgery in the Era of COVID

I have many patients and friends asking me, “Is it safe to have cosmetic eyelid surgery, Botox and filler in COVID times?” My universal answer is, Yes! Now is a great time to have cosmetic eyelid surgery. In fact, I have seen an increase in patients coming in to discuss improving the appearance of their eyelids and having surgery.

In some ways, this is a perfect time to have cosmetic eyelid surgery. There are no parties, events or in-person work commitments. Most vacations are being taken at home or only a car ride away. So taking a week or so off to recover has never been easier.

I am seeing many patients in the office that also have been looking at their eyes on Zoom calls and now are finally ready to do something about their eyelid bags or tired-looking eyelids. Also, as the rest of their face is covered by a mask, the eyes are now truly the only facial feature that others are seeing. So everyone wants them looking their best.

My patients have truly appreciated all the steps I have been taking in the office and in my private surgery center to ensure their safety. We have all the usual precautions: temperature checks, screening questions, social distancing and thorough cleaning between patients, etc. But we also have our entire staff COVID tested once a week, and all patients are tested before surgery. With these precautions we have maintained a COVID free environment in our office and surgery center.

It has given me great joy, especially in the era of COVID, to be in the business of making my patients look and feel better.

Cosmetic Eyelid Surgery In Men

Cosmetic eyelid rejuvenation for men has always been a part of my surgical practice. I see men of all ages who want to optimize the appearance of their eyes and upper face. In this blog I will go by age group and describe the most common procedures and how they are tailored for the male aesthetic.

20’s-30’s

Men in this age group will often be reaching out to me for cosmetic evaluations for the first time. Usually I help them achieve their goals with neurotoxins (Botox, Xeomin, Dysport, Jeuveau) and fillers. Cosmetic blepharoplasty is not that common in this age group, but sometimes I will treat lower eyelid bags with a lower eyelid blepharoplasty. For men in their 20s and 30s I almost always approach these bags through a transconjunctival approach. This means that the incision is on the inside of the eyelid so there are no visible scars.

A common surgery I do for men in their 20s and 30s is the repair of congenital ptosis. This is a condition when a child is born with a droopy eyelid which stays droopy as they grow. Often only one eye is affected but sometimes both lids can be droopy. If the droop is severe enough to interfere with the development of vision then it is always repaired in the first few years of life. However, if the droop is mild, then it is usually not repaired until later.

Men in their 20s and 30s often come into my office for repair of a mild droop. Thes surgery to repair this droop or “Ptosis” is simple and done as an outpatient procedure. I tighten the muscle that opens the eye to give it more pulling power. I do this from the backside of the eyelid so there is no incision on the skin and no visible scar.

40’s-50’s

Men in this age group often come to see me for non-surgical treatment of aging changes. This includes softening wrinkles with neurotoxin (Botox, Xeomin, Dysport, Jeuveau) and/or using fillers to soften contour issues. These non-surgical procedures are done in the office with no down time.

I also see men in this age group who have aging changes of the eyelids that need surgery. Most common are lower lid bags. Some men develop lower lid bags even in their 20s and 30s. Often there is a family history of developing early eyelid bags. In many cases I can approach these bags through a “trans-conjucntival” approach. This means that the incision is on the inside of the eyelid so there are no visible scars. I find this a very nice option, especially for men.

50’s and beyond

In this age group I treat patients with neurotoxins (Botox, Xeomin, Dysport, Jeuveau) and fillers, but surgery is often what brings patients first into my office. Heavy upper eyelids and lower lid bags are common complaints. I commonly perform upper and lower eyelid blepharoplasty. In men I take extra care to keep the eyelid looking “masculine.” This is achieved by the placement and shape of the upper eyelid lid crease and also carefully tailoring how much skin I remove from the upper eyelids.

The modern aesthetic for eyelid lifts is a natural look. Patients want to appear refreshed after surgery, not pulled and tucked. The biggest concern I hear from my patients is that they will look “done.” So how can you be assured that your result will be just right?

First it’s important to find a surgeon that understands your aesthetic sensibility. I tell my patients that their surgery is all about them and their goals. I want them to see me as a tool to guide them and help them achieve exactly what they are hoping for. I find it is essential to listen closely to my patients, so I can be sure to completely understand what they want to achieve with their surgery.

In our initial consultation I explain what we can achieve with the various surgical procedures that I offer. Whether it is a cosmetic blepharoplasty, endoscopic brow lift or a ptosis repair, it is important for the patient to understand how surgery will improve their appearance. It is often helpful to look at before and after photos of patients that I have operated on. This helps to envision what the surgery will achieve.

I thought it would be helpful to list some of the most asked questions I get on a day to day basis when consulting with new patients about cosmetic eyelid surgery. Here are a few and more to follow in future blog posts:

How long does it take to recover from an upper eyelid blepharoplasty?

For most patients, an upper eyelid blepharoplasty is a relatively easy procedure and recovery. There is some slight discomfort after the surgery. Most of my patients manage the discomfort with Tylenol. The day of the surgery you will need to ice for about 20 minutes per hour, but otherwise you can be up and about at home. You can be on your phone or watching TV. Stitches come out one week after surgery. You will have some mild bruising and swelling that will be mostly gone after 7 days.

What type of anesthesia is used for cosmetic blepharoplasty?

For a cosmetic blepharoplasty there are several choices for anesthesia. In my private surgical center we offer light IV sedation to general anesthesia. For the upper eyelids, some patients choose to be completely awake and others prefer IV sedation. When I am performing a lower lid blepharoplasty, general anesthesia is used to keep my patients safe and comfortable.

What results can I expect after a cosmetic blepharoplasty?

You can expect your eyes to look refreshed, less tired and completely natural.

What is the best time of year to have cosmetic eyelid surgery?

That is a personal decision. Some of my patients like the winter months, because they are less active with outdoor activities and can more easily take the time to recover. Other patients like the summer months because their kids are away at sleep away camp and they have fewer scheduled activities.

Laser skin resurfacing is a great adjunct to cosmetic blepharoplasty

Laser skin resurfacing is a great adjunct to cosmetic blepharoplasty. It rejuvenates the skin and stimulates collagen. The laser can be done at the same time as surgery so that there is one healing time. The laser can also be done on its own.

In my office center, I use the Lutronic eC02 fractional laser. This laser works by creating tiny micro wounds, leaving bridges of healthy skin, thus treating a “fraction” of the skin’s surface. This allows quick healing times with great results.

The laser reduces imperfections in the skin and promotes collagen regeneration for long term wrinkle reduction. It also improves the tone and color of the skin, giving it a smooth, even glow.

Typically only a single treatment is needed. The Co2 laser:

Reduces facial lines and wrinkles
Gives the skin a more firm, smooth appearance
Improves appearance of scars
Reduces sun damage such as brown spots
Improves appearance of pigment and vascular conditions

Immediately after the treatment, you will have some swelling and redness. This usually subsides in 5-7 days depending on how aggressive the treatment was. Results are seen after a few days when the skin will look smoother and more even toned. The skin will continue to improve over the next 6-9 months as new collagen builds.

Here are a few more of the most frequently asked questions about cosmetic eyelid surgery:

Is cosmetic eyelid surgery painful?

No, most patients experience mild discomfort but do not describe pain. We will give you medication for pain, but most of my patients don’t need it.

Does eyelid surgery leave scars?

There is always a scar after surgery. However the eyelid skin heals the best out of any area on the body. This is because the eyelid skin is thin. So while there are very faint scars, they become undetectable over time.

How long will my eyes be blurry after surgery?

Most people experience a small amount of blurry vision for a day or two after surgery. This can be treated with lubricating drops. You will have no bandages, and you will be able to see immediately after the surgery. The blurriness is mostly when reading fine print.

How long does eyelid surgery last?

Eyelid surgery typically lasts a lifetime. Over the years, the face will continue to age. Care with sun exposure is a good idea to keep the skin and underlying collagen healthy. Some patients may find that their brows drop over time, and may elect to do a brow lift years after their eyelid surgery.

Choosing To Have Cosmetic Eyelid Surgery?

My patients undergo cosmetic eyelid surgery for many reasons. Most are trying to look younger, less tired, more symmetric or just overall refreshed. The decision is personal.

Cosmetic eyelid surgery won’t change your life. It won’t solve personal problems or make you look like someone else. But it can give you greater self-confidence and add to your sense of well-being.

Good questions to ask yourself before undertaking cosmetic eyelid surgery are:

What is your motivation? Are you doing it for someone else, or for yourself?
What do you want to change and why?
How long have you wanted to do this?
What are your expectations?

When patients come in with the answers listed below, I know they are well on their way to being ready for cosmetic eyelid surgery:

“I want to do it for myself.”
“I look into the mirror and I don’t recognize that person.”
“I feel young, I exercise, but I don’t look the way I feel.”
