									<p>As an oculoplastic surgeon I spend many days talking to patients about how to improve the appearance of their eyelids. This can be a confusing topic. Patients often come to the office saying that they look “tired” all the time but don’t know exactly how to fix the problem. They tell me that no matter how much they sleep, they still wake up with dark circles and a tired appearance.</p>
<p>A tired appearance can be caused by several different issues. In this blog I am going to focus on the lower eyelids. Under eye bags and dark circles make the whole face look tired. No amount of sleep will help to improve the bags. Also, loose and wrinkled lower eyelid skin can give a “tired” appearance.</p>
<p>The non-surgical treatment for lower eyelid bags and dark circles is to use an <a href="https://www.drjessicalattman.com/non-surgical/injectable-fillers/">injectable filler</a> to smooth out the contour of the lower eyelid. I use Restylane in the lower eyelids. Filler can help to improve the couture of the lower lids and remove dark circles. Not everyone is a good candidate for lower eyelid filler. So it is important that I evaluate all aspects of the eyelid before choosing the best treatment.</p>
<p>The surgical treatment for under eye bags and dark circles is a <a href="https://www.drjessicalattman.com/cosmetic-procedures/lower-blepharoplasty/">lower eyelid blepharoplasty</a>. In this procedure I remove and reposition fat pockets in the lower eyelid to improve the contour of the lid. If needed, I also tighten the deeper layers of the eyelid and trim the skin to give a smooth appearance to the eyelid. Often I will also resurface the eyelid skin with a <strong>CO2 laser</strong> to smooth out fine lines and improve the elasticity of the skin.</p>

									<p>As an oculoplastic surgeon I often get asked the question, “What do I do about my under eye dark circles?” While there is not a “one size fits all” answer, here I hope to expand your understanding of the problem.</p><p>For most, dark under eye circles are caused by the contour of the lower eyelid. As we age, the fat and collagen in the lower eyelid shift, which can cause a hollow depression. This depression in the lower lid can cause overhead light to cast a shadow that appears as a dark circle.</p><p>A good way to see if your dark circles are from a contour problem is to take a flashlight and shine the light from below. If the darkness improves then you most likely have a contour issue.</p><p>For some, the under eye skin is more has more pigmentation and this is what causes the dark appearance. Also as we age the skin of the eyelid becomes thinner, allowing the darkness of deeper structures in the eyelid to show through and give a dark hue.</p><p>There are many great solutions to lower eyelid hollows. Filler is a great option for patients in their 20s to early 40s who have a small hollow area. <a href="https://www.drjessicalattman.com/non-surgical/injectable-fillers/">Filler</a> is done in the office and most patients go right back to normal after the injection. Some mild swelling and bruising can occur.</p>								</div>

									<p>Pictured here is a 57 year old woman who came to see for help with the appearance of her eyes. She felt that she looked tired all the time despite getting good sleep and taking care of herself. She noticed drooping of her upper eyelids, excess skin and dark circles on her lower lids.</p><p>In the pre-op photo you can see an extreme heaviness of her upper lids. Her eyebrows are also low. The combination of all this extra weight on her eyelids, have pushed the lids down so she looks half asleep. Also in the pre-op photos you can see that her lower lids have bags and excess hanging skin, causing dark circles and a tired appearance.</p><p>4 months ago I performed and upper and lower blepharoplasty and an endoscopic brow lift. As part of the lower eyelid blepharoplasty, I performed my signature “muscle suspension.” This allows a complete rejuvenation of the lower lids without changing the shape of the eye. The surgery took about 2 hours and the recovery a few weeks.</p>								</div>

I see men of all ages who want to improve their appearance in a very natural way. Eyelid surgery is one of the most common procedures that men today are getting done.</p><p>It is especially important for men to be in the hands of an oculoplastic surgeon who understands the unique anatomy of the male eye and the important nuances that will keep the results looking natural. I take a much different approach to male eyes, than I would for female eyes. This is to ensure that the results maintain a “masculine” looking eye and are completely undetectable right after surgery.</p>								</div>

									<p><strong>A:</strong> In the transconjunctival approach, the incision is made on the inside of the eyelid, so that there is no scar and no stitches. This is an excellent choice for younger patients who only have eyelid bags without excess skin and sagging muscle. The advantage of this approach is that the incision is made on the inside of the eyelid, so the recovery is much faster. The disadvantage is that sagging skin cannot be corrected with this approach. I will often combine a transconjunctival blepharoplasty with a laser resurfacing to the lower eyelids. The laser helps to improve the quality of the skin and reduces fine lines and wrinkles.</p><p>In the subciliary approach an incision is made just below the eyelashes. This allows a more aggressive solution for patients who have sagging skin, excess rolls of muscle, or skin and muscle that are hanging.</p>								</div>

									<p><strong>A:</strong> While your lower lids will continue to age with the rest of your face, the bags that are removed typically do not come back. I have been in practice for 21 years, performing lower lid blepharoplasty, and I have seen my patients results age naturally over the years. It is very uncommon to repeat a lower lid blepharoplasty once it is done.</p>								</div>

Most patients are back at work within 4–5 days, sometimes sooner.  With the subciliary approach patients have stitches for a week, and then usually some residual swelling for another few days after the sutures are removed. Pictured here is a 54-year-old man who was bothered by his lower eyelid bags and heavy upper eyelids. He came to see me wanting to improve the way he looked.</p><p>I explained to him during his consultation that the best option for him was upper and lower lid blepharoplasty. This surgery would give him the improvement he was looking for, with a long-lasting and natural result.</p><p>On the day of surgery, we spent time together, before going into the operating room, discussing again exactly what I was going to do and what he wanted to achieve with the surgery. Then I carefully marked his eyelids. I use these markings as a guide during the surgery.</p><p>After surgery he went home with a private duty nurse for one night. He was able to start working from home after a few days. He came into the office a week later to have the sutures removed and was back in the office the following day.</p>								</div>

									<p>Lower eyelid blepharoplasty can be approached in two ways: transconjunctival (from the inside of the eyelid) and subciliary (from an incision just below the eyelashes on the outside of the eyelids). The transconjunctival (TC) approach is a great option for patients who only have lower lid bags, without loose or excess skin. A lower lid TC blepharoplasty can be combined with a laser skin resurfacing treatment for maximum effect.</p><p>The lower eyelids are the most complicated area of the face. They have important function—to protect the eyeball—also have a complex anatomy. As an oculoplastic surgeon, I have a great understanding of the eyelid anatomy and how it functions to protect the eyeball. In my experience, it is essential to choose a surgeon who has a deep understanding of the eyelids and who performs lower eyelid surgery as a regular part of their practice.</p><p>I personally love the TC approach for young patients who have lower lid eye bags. I see a lot of young people who have eye bags that started to show up, sometimes as early as their late teens. These patients are excellent candidates for the TC approach.</p>								</div>

The lower eyelids age in many different ways and can show signs of aging at different ages. I see young patients who develop lower eyelid bags in their 20s and I also see some patients in their late 70s that are exploring plastic surgery for the first time.</p><p>Most patients start to notice in their 30s and 40s changes in their lower eyelids that they don’t like. These changes can include bags, wrinkles, hollows, and dark circles. Lower lid blepharoplasty is an excellent way to address these changes.</p><p>Lower lid blepharoplasty is an outpatient procedure that is done in my private surgical suite. You go home the same day, and recovery is relatively quick and easy. There are two approaches for the lower lids. One is called a subciliary approach, which involves an incision just under the eyelashes. The other is a transconjunctival approach. where all the work is hidden and done from the inside of the eyelid. I will discuss both in detail with you and will choose the approach that best fits your goals and lifestyle.</p>								</div>
