This 54-year-old woman had been bothered by her under-eye bags for years. She finally couldn’t take them anymore and decided to have cosmetic eyelid surgery. I performed an upper and lower eyelid blepharoplasty on her 6 weeks ago. She is absolutely thrilled with her new eyelids.

I used a subciliary approach to do the surgery of her lower lids. This allowed me to recontour the lower eyelids and also tighten the skin and deeper muscle to give her a perfectly smooth lower eyelid with a natural look.

Help, I Look Tired All The Time!

As an oculoplastic surgeon I spend many days talking to patients about how to improve the appearance of their eyelids. This can be a confusing topic. Patients often come to the office saying that they look “tired” all the time but don’t know exactly how to fix the problem. They tell me that no matter how much they sleep, they still wake up with dark circles and a tired appearance.

A tired appearance can be caused by several different issues. In this blog I am going to focus on the lower eyelids. Under eye bags and dark circles make the whole face look tired. No amount of sleep will help to improve the bags. Also, loose and wrinkled lower eyelid skin can give a “tired” appearance.

The non-surgical treatment for lower eyelid bags and dark circles is to use an injectable filler to smooth out the contour of the lower eyelid. I use Restylane in the lower eyelids. Filler can help to improve the couture of the lower lids and remove dark circles. Not everyone is a good candidate for lower eyelid filler. So it is important that I evaluate all aspects of the eyelid before choosing the best treatment.

The surgical treatment for under eye bags and dark circles is a lower eyelid blepharoplasty. In this procedure I remove and reposition fat pockets in the lower eyelid to improve the contour of the lid. If needed, I also tighten the deeper layers of the eyelid and trim the skin to give a smooth appearance to the eyelid. Often I will also resurface the eyelid skin with a CO2 laser to smooth out fine lines and improve the elasticity of the skin.

How do I get rid of dark circles under my eyes?

As an oculoplastic surgeon I often get asked the question, “What do I do about my under eye dark circles?” While there is not a “one size fits all” answer, here I hope to expand your understanding of the problem.

For most, dark under eye circles are caused by the contour of the lower eyelid. As we age, the fat and collagen in the lower eyelid shift, which can cause a hollow depression. This depression in the lower lid can cause overhead light to cast a shadow that appears as a dark circle.

A good way to see if your dark circles are from a contour problem is to take a flashlight and shine the light from below. If the darkness improves then you most likely have a contour issue.

For some, the under eye skin is more has more pigmentation and this is what causes the dark appearance. Also as we age the skin of the eyelid becomes thinner, allowing the darkness of deeper structures in the eyelid to show through and give a dark hue.

There are many great solutions to lower eyelid hollows. Filler is a great option for patients in their 20s to early 40s who have a small hollow area. Filler is done in the office and most patients go right back to normal after the injection. Some mild swelling and bruising can occur.

Lower eyelid blepharoplasty is a great option for patients who are in their 30s and up. Surgical blepharoplasty corrects contour issues, tightens the skin and deeper structures to give a natural youthful eyelid.

I perform my browlifts with an endoscopic approach. This is done through 4 small incisions behind the hairline. I use an operating telescope to lift the brow through these small incisions. Recovery is quick. The results are natural, and in the right patients, an endoscopic browlift is an important part of rejuvenating the upper eyelids.

NYC Male Blepharoplasty

I often get asked what percentage of your patient’s are men. Today about 20% of my patients are men. Over my 22 years in practice I have seen the number of men coming in for eyelid surgery and other non-surgical treatments rise. I see men of all ages who want to improve their appearance in a very natural way. Eyelid surgery is one of the most common procedures that men today are getting done.

It is especially important for men to be in the hands of an oculoplastic surgeon who understands the unique anatomy of the male eye and the important nuances that will keep the results looking natural. I take a much different approach to male eyes, than I would for female eyes. This is to ensure that the results maintain a “masculine” looking eye and are completely undetectable right after surgery.

Case Study Upper Eyelid Blepharoplasty and Endoscopic Browlift

Pictured here is a lovely 61-year-old woman who came to me bothered by the appearance of her upper lids. She had done some research online and was pretty sure that she needed an upper eyelid blepharoplasty.

Together, we looked at her upper eyelids and brow position. If you take a look at her pre-op photo, you can see a distinct heaviness in the outer corners of her upper eyelids. This is caused by a drooping of the brow. When I gently raised her brow with my fingers, she loved how her eyelids looked. So, we agreed to proceed with an endoscopic brow lift and upper lid blepharoplasty.

This post-op photo is one month after surgery. She is thrilled with her new eyelids. By lifting the brows, I was able to give her a natural look with a quick recovery time.

Endoscopic brow lifts can sometimes feel overwhelming to patients who come in to see me for their heavy upper lids and have not considered the position of the eyebrows. However, the endoscopic brow lift is a great procedure, which predictably restores a natural brow position. This gives a clean look to the upper eyelids, with a quick recovery time.

Case Study- Upper Blepharoplasty and Endoscopic Brow Lift

I love this set of before-and-after photos. They are of a 47-year-old mom of five. She came to me looking for a solution to the heaviness of her upper eyelids. She had done some research online and thought that she needed an upper lid blepharoplasty to help with the excess skin hanging over her upper eyelids.

I met her for consultation and went over what her goals, were and carefully examined her upper face and eyelids. What became apparent was that she was a great candidate for both an upper lid blepharoplasty and an endoscopic brow lift. Over time her brows had dropped, causing extra skin to hang past the outer corners of her eyes. The most natural way to smooth the heaviness of the outer corners of her eyelids was with an endoscopic brow lift combined with an upper lid blepharoplasty.

This was truly life-changing for my patient. She never thought that her eyes could look so bright, and open and yet so natural at the same time.

Upper and Lower Blepharoplasty Case Study

I am excited to share my first “Case Study.” I believe that by writing about specific cases, it will help my current and future patients feel more comfortable with the process of eyelid surgery. Pictured here is a 54-year-old man who was bothered by his lower eyelid bags and heavy upper eyelids. He came to see me wanting to improve the way he looked.

I explained to him during his consultation that the best option for him was upper and lower lid blepharoplasty. This surgery would give him the improvement he was looking for, with a long-lasting and natural result.

On the day of surgery, we spent time together, before going into the operating room, discussing again exactly what I was going to do and what he wanted to achieve with the surgery. Then I carefully marked his eyelids. I use these markings as a guide during the surgery.

After surgery he went home with a private duty nurse for one night. He was able to start working from home after a few days. He came into the office a week later to have the sutures removed and was back in the office the following day.

This post-op photo was taken one month after surgery. He is mostly healed, but some slight swelling and redness may continue to resolve over time. He was absolutely thrilled with his result and the entire experience.

How do I know if I am a good candidate for an upper lid blepharoplasty?

When I first meet a patient in consultation, I always ask, “What is bothering you?”

If their answers are like any of the below, they are probably a good candidate for an upper lid blepharoplasty.

“My eyelids are drooping.”
“My upper eyelids are sagging”
“I can’t put on makeup anymore without it smudging.”
“I look tired all the time.”
“I lost my smooth-looking eyelid.”

The next step is a thorough exam of the eyelids and the eyes themselves. As an oculoplastic surgeon, I trained as both an ophthalmologist and a plastic surgeon. So, I examine all my patients’ eyes to look for ophthalmic issues that may complicate surgery.

Some of these conditions include dry eye, blepharitis or poor closure of the eyelids. The health of your eyes is incredibly important—especially when undertaking eyelid surgery. If I find any problems, I will treat these before proceeding with surgery.

Next, I examine the eyelids and show you what you can expect from the surgery. I can show you an approximation of what the eyelids will look like. Then, I will show you photos of patients whom I have already operated on (of course, only those who have given permission to share their photos). It is very important to me that my patients know exactly what to expect after surgery.

Q: What is it like on the day of surgery for an upper lid blepharoplasty?

A: Before scheduling the procedure, I will discuss with your options about where you would like to have it done. There are two options:

In the operating room (OR)
In the office

If you are medically well, then the office is a great choice for an upper lid blepharoplasty. You will come into the office, after eating a normal breakfast. When you get to the office, you have the option of taking a medication to help you relax; this is a pill—no IV or needles required. If you choose to have the procedure in the OR, you will come to my surgical center and receive IV sedation by our anesthesiologist.

I will come into the room, and we will go over the surgery, and I will mark your upper eyelids with a surgical marking pen. Then you will lie flat in the procedure chair, and I will inject a small amount of lidocaine into the upper eyelid skin to make it numb.

The nurse will clean your face to make it sterile and place sterile drapes around your head.

Then I will start the procedure. Once I start, it takes about 30 minutes of actual operating time. All you have to do is relax. We can play music or talk—whatever makes you most comfortable. If you have IV sedation in the OR, you will take a short nap and wake up when it is over.

When I am done, you will sit with ice compresses on your eyelids for about 20 minutes, and then someone will need to come and pick you up.

Most patients have little to no pain after the surgery. Upper lid blepharoplasty is a great way to get rid of upper lid heaviness and hooding. You get a fresh, new set of eyelids!

Upper lid Blepharoplasty In The Office Setting

An upper lid Blepharoplasty is a surgery that removes excess skin and “baggage” from the upper lids. It restores a rested and youthful look to the eyelids. I often see patients in my practice who are unhappy with the heaviness and tired look of their upper eyelids, but they are not sure if they are ready for surgery.

In consultation, we look closely together at their eyelids and discuss what they want to achieve. If your goals are in line with what surgery will provide, then we go forward. Sometimes, Botox or other procedures can address the problem, and I will offer these treatments as a first step.

So, if you are unhappy with the look of your eyelids, and are not sure what to do, the best first step is to come into the office for a consultation. Then together we can look at your eyelids, talk about your goals, and find the procedure that fits best.
